And just to clarify, what makes the incremental improvements feasible is that you have a battery of tests to give you some confidence that you're not breaking existing functionality. If you find yourself thinking "I can't change that" then you've just identified somewhere that needs test coverage.

I always like the humble approach, I would reject the change on the basis that I don't understand and I'm therefore, not qualified to give it the ok.

I was going to make the same suggestion. The boss doesn't want a conceptual overview, he wants it line by line so it should be possible to crank out something in a day or so that creates utterly useless but superficially correct documentation. It wouldn't matter if it were a rats nest of if-elses in perl that was easily confused by corner cases, it would just identify variable declarations, modifications to variables, method calls etc (remember that it's line by line, it in no way groks the code).
