@Lyise, Thank you for your remark. You are absolutely correct. I should incorporate your good point when I get a chance.

comment Help with database design with a strict and structured subset of clinical data
"I picked mongodb, because I'm afraid the requisites for this database will be changing from time to time" - I disagree with this. I belive in schema-first approach to developing business applications. If business changes, business pays.

comment End user query syntax?
You have a point. A SELECT can be harmful (e.g. stackoverflow.com/questions/444248/…). However, you can protect the database in several ways such as (a) control user privileges and/or Create views that don't allow updates and let the user use the views not the tables (this may be controlled by privileges). If the data is static after loading, you may be able to make the entire database read only but this depends greatly on your database engine.

comment End user query syntax?
Just wondering, what is statically-schemed? Does it consist of 1 or more tables? Why not simply analyze the user entered SQL Query string and ensure it starts with a 'Select' if you only want queries only?

comment What must I take into consideration when designing a UI around a 0..1:1 relationship?
I disagree with this approach in general. Separating optional fields will not solve the different null interpretation problem. If you have 100 tables with optional columns will you be adding another 100 tables of optional columns?
