You can have defects in compilers, operating systems, libraries, CPUs, microcode, memory, disk controllers and the initial program specifications. Add in bit-flips caused by occasional cosmic rays and we can see that life is hard.

There is "formally verified" software. It is mathematically proven to work. Occasionally even formally verified code has bugs. IIRC Java's quicksort implementation was formally verified, but that did not account for overflows.
