See what the market needs. I wouldn't recommend doing random certificate courses and then trying your luck. If you already have clients, you shouldn't have a problem upgrading your skills. Ask them for feedback and decide what more they need from you.

You should learn to mix programming with lots of physical activity. Take to football or hiking, these are better than the anti-depressants you've been relying on.
